Minnesota DHS - HHAeXchange Provider Enrollment Form
If you have not already received your client portal and API information from HHAeXchange, you will need to complete the Minnesota DHS - HHAeXchange Provider Enrollment Form.

As you fill out this form, there are a few important items to note.
Step 3 will ask you to input your agency locations & criteria. If you have multiple NPI/UMPI's, make sure to add each while completing — using the `Add Agency NPI/UMPI` button to do so.
Step 5 will ask if you are currently using an EVV system. The answer to this question is:
Yes - I currently have my own EVV system and would like to integration with HHAX (EDI)
Within step 5, once you've selected that you have your own EVV system that you would like to integrate, additional fields will appear asking for information about your current EVV system. These fields and the information to use for them are below.
