Incident Reporting

Incident reports can be made in back-office URL and via our Mobile applications.

Once a Caregiver or a Clinician completes an Incident Report in their Mobile Application, the Back Office will have access to these reports. Similarly, incident reports that are created on the computer will also appear in the Billiyo apps.

How To Create Report an Incident in the Back Office

First navigate to OPERATIONS > Incident Report > Report an Incident

This will take you to a blank form where you can  fill out the incident report. After creating a report, make sure you sign your name as either a Supervisor or Employee.

Finally, click on Submit to save your incident report.

You can also create an Incident Report in the Patient Dashboard/Profile. To do this enter your client's profile or dashboard and scroll to the bottom of the page. In the tabs section there is a tab called "Incidents" click into this tab to see all incidents that have been prepared for the client. To add a new incident report, hit the grey plus sign to the top right of the table.

 

You will then be able to prepare an incident report for the chosen client.

Once there is an incident report created you have options.

Click on the button.

Once you click on the button, a drop down will appear.

Here you can:

  • View PDF

  • eFax (only available for our communications package subscribers)

  • Mark as Sent

  • Mark as Signed and Received

  • Compose Email

 

 

How to View an Incident Report in the Back Office

First navigate through: Operation > Incident Report > View Incident Reports

 

Then use the filters at the top of the  page to locate the incident report you want to work with.

After finding your incident report, you can view it more details by clicking on the Green_Plus. This will expand the incident and show you more information.  To revise an incident report select Edit Incident Report and to delete one click Delete Incident Report.

Click on the button.

Once you click on the button, a drop down will appear.

Here you can:

  • View PDF

  • eFax (only available for our communications package subscribers)

  • Mark as Sent

  • Mark as Signed and Received

  • Compose Email

You can also view an incident report from within the client's profile. To do this enter your client's profile or dashboard  and scroll to the bottom of the page. Incident Reports in the app

To report an Incident, or to view one that has already been reported, we can also use the Clinical/Caregiver Application:

In the Clinical App Navigate to the "Clinical" tab and in the Caregiver App go to "More".

 

Then locate the "Incident" tile and tap on it.

You can also navigate to the Incident report by clicking on the Notification that you have received in the application via the notifications page in "More" tab. Simply tap on the notification tile and then select the incident notification.

 

 

To build and incident report in the app tap on the  plus sign to the bottom right of the page:

If you can't see the incident report you are looking for then you may need to adjust the filters to the top right of page.

To edit or sign an incident simply tab on the report you want to work with. This option is available when the Caregiver is the employee "involved" is chosen by the Incident Report Writer, rather it is in the back office or via the Clinical Application. 

Click on it, sign it and hit the check mark at the top of the page. 

There is also the PDF icon here, you can view the PDF version of the Incident report and once opened in PDF format, you can also print it from here. 

 

 

Incident Report Status

When an Incident report is created in the Back-Office URL and Signed by a Supervisor, the status of the Incident Report will automatically change from Open to Closed.

If the Incident Report was Created via the Mobile Application, it will be in an "Open" status, until a back office user has come to the View Incident Report Page or the Patient Dashboard and clicked on the edit button and changed the status to "Closed".

In order to Mark and Incident Report as "Closed", it must be signed by a Supervisor.

Once an Incident Report is closed you will not be able to edit it.

You can add an addendum to the Incident report by clicking on the Green Plus sign of the Incident Report.

Click the Blue Addendum.
