Incident Reporting

Incident reports can be made in back-office URL and via our Mobile applications.

Once a Caregiver or a Clinician has completed an Incident Report via the Mobile Application, the Back Office will have access to these Incident Reports and Vice Versa.

 

To Report an Incident in the Back Office

1. From the Main Menu, selecting the following:

OPERATIONS > Incident Report > Report an Incident

2. Fill out form

3. Sign as the Supervisor or as the Employee that is taking the report in the Back office

4.Select Submit

You may also chose to add an Incident Report via the Patient Dashboard Page.

This Feature is available in the Incident Tab at the Bottom of the Page: click on the Tab and click on the Plus sign on the Right hand side of the Page.

To View an Incident Report in the Back Office

1. From the Main Menu, select the following:

OPERATIONS > Incident Report > View Incident Reports

2. You can use the search filters

3. Click on the Green_Plusfor details of the incident 

4. If anything needs to be modified, select the Edit Incident Report. You may also delete the report by click Delete Incident Report which you will be prompted to verify the deletion.

You may also view incident reports via the Patient Dashboard.

They are located in the bottom of the page in the Patient Dashboard in the Incidents Tab.

This Page has the Same functionality as the View Incident Report Page

When an Incident report is created in the Back-Office URL and Signed by a Supervisor, the status of the Incident Report will automatically change from Open to Closed.

If the Incident Report was Created via the Mobile Application, it will be in an "Open" status, until a back office user has come to the View Incident Report Page or the Patient Dashboard and clicked on the edit button and changed the status to "Closed".

In order to Mark and Incident Report as "Closed", it must be signed by a Supervisor.

Once an Incident Report is closed you will not be able to edit it.

You can add an addendum to the Incident report by clicking on the Green Plus sign of the Incident Report.

Click the Blue Addendum.

 

To report an Incident or to view one that has been reported on a client that you are connected with, Via the Caregiver Application:

Go to More at the Bottom Right of the page. 

(You may also navigate to the Incident report by clicking on the Notification that you have received in the application via the notifications page in More Tab)

At the bottom Right of this page there is a Plus sign, Click here to report a new Incident.

To View other Incidents that you may have received a notification for, you may use the Filter option at the top Right of the page to view all clients, or a specific client and choose a date range to search by. 

This is where you as a Caregiver will sign the incident in order to acknowledge that you are aware of it, if it was created by another User.

This option is available when the Caregiver is the employee "involved" is chosen by the Incident Report Writer, rather it is in the back office or via the Clinical Application. 
Click on it, sign it and hit the check mark at the top of the page. 

There is also the PDF icon here, you can view the PDF version of the Incident report and once opened in PDF format, you can also print it from here.

This option is available when the Clinician is listed as the employee "involved" or is chosen as the Supervisor by the Incident Report Writer, rather it is in the back office or via the Caregiver Application.
