      Supervisory Visits and Care Giver Evaluations

      These Forms are located in the Clinical Forms Page. Here you can view Forms that have been submitted by QP's on the Clinical Application.

      Navigation: To View Clinical Forms head on over to Clinical > Clinical Forms > View Forms

      Here you can Filter by Client, Employee, Clinical Form Type, Submission Date and Archived or unarchived.
      Make Sure to Click your Blue Refresh Arrows once you have made your Selections.

      For these Two Types of Clinical Forms, the Page includes Analytics to help you manage your compliance.

      In the Clinical Forms Type Filter Option's, choose Supervisory Visit or Caregiver Evaluation as the Form Type. 

      This will open Analytics for you to see the Number of Upcoming or Past Due Forms.

      Supervisory Visits and Caregiver evaluations that are due or coming to be due will show here based on the Company Settings for these two Forms:

      • If the company settings flag is turned off, the system will default to 30-day, 60-day, and 90-day ranges.

      • If the Supervisory Visit Settings or Caregiver Evaluation Settings are configured, it will display the Clinical Forms Page accordingly, based on the selected intervals."

      You can Click on each one for the Various Number of Forms Due for example "45 days" and a Page will open to show you the details for these Upcoming or Past Due Forms between 15 and 45 days. 

      And you can turn it into a Report by Clicking on PDF and CSV.

      To Take Action on a Form simply Click on the Gear Icon in the Action Column.

      Here you can:

      • View the Clinical Assessment
        • This will take you to a Page where you can make Edits to the Form
      • View the PDF
      • View the Full PDF
      • eFAX 
        (note: to use the Billiyo eFax Feature, you must have the Billiyo Communications Package in your Agency Subscription)
      • Or Archive the Form
        • This has replaced the Delete option and will be Used for a Form that was created in Error.
