Adding and Managing a Diagnosis Code

There are a few different places that a diagnosis code can be added for a client.

In the Client Profile


Within a client's profile, their diagnosis code(s) can be added. To view a client's profile, head to CRM > Clients > View Clients. Next, enter your client's profile and click on "Diagnosis" tab to the left of the client's name.





A new tab will appear that shows the diagnosis codes that have been entered for this client.

To add a new diagnosis, please hit the blue plus sign . Then, in the new fields that appear, please fill out the diagnosis you are entering.


  • Diagnosis Type - Choose what type of diagnosis we are adding.
    • Principal Diagnosis - The "main" diagnosis a client has
      • Each client can only have one principal diagnosis
    • Surgical Diagnosis - A diagnosis that comes from a surgery
    • Other Diagnosis - Any additional diagnosis codes a client may receive
      • If a client gets a "new" principal diagnosis, then their previous principal should be changed to "other."
  • Start Date - When the diagnosis was given
  • End Date - When the diagnosis ends
    • This field can be left blank for diagnoses that do not end 
  • ICD-10 - Diagnosis Code - The ICD-10 code we are entering
    • If you cannot find the ICD-10 code you need, please contact your Customer Success Manager 
After selecting the diagnosis code, click the Save button. 


Edit Diagnosis Code

To edit a diagnosis after entering it, we can hit the blue pencil icon.

This will trigger a pop up where we can edit the diagnosis that was entered.

To resequence your diagnosis codes, we can click on on the blue sequence icon. In the resulting pop up, you can drag and drop your diagnosis codes in order to set their priority. 

You can only resequence "Other" Diagnosis types.

 

 



In the Service Authorization

When adding a service authorization, the client's diagnosis code can also be added. To add a service authorization, head to CRM > Service Auth > Add/View SA.

During the service auth entry process, either manual or automated, there will be a field for diagnosis code. 


If the diagnosis code is not appearing when you type into field, then we will need to add it. To do this, please click on the ellipsis. 


This will trigger a pop up where you can enter a new diagnosis for the client.

The fields we see when adding a diagnosis code through the Service Auth are the same as the fields we see when adding a diagnosis through client profile.

Make sure to click Save after finding the diagnosis code.
